In my opinion...all of them help towards the overall success of a site.
I personally see better results with good body content (a good mix of information with main key phrases appearing appearing at a ratio of about 2-10%, depending on the content).
The page title is without doubt the most important part of the 'off the page' content. This is what the search engines display when you do a search, so get those keywords in there (dont over stuff tho).
The Keyword meta-tag helps a little bit, tho most of the main search engines pay little attention to it. But try to only optyimise for between 1 and 3 keywords/phrases per page. Too many and you will dilute their importance.

I'm really new to this SEO thing, but so far, i've come across pages that have had little to do with the search term I put in, but they get listed really high. The one thing that stood out was the overall content on the page... which was massive.
Now, thats not to say it will work, but Google is known for favoring pages with a lot of content.
On the other hand, I did come accross one page once that had a PR of 3, and it had one very large image, and three links... that went to another single page, that also had one large image (total crap for content). I think the page wasn't intended to be that way, but the person just sucked at making pages. So in this case, I think it had something to do with 'time on the web', 'title', 'description', and whoever else linked to him... which I don't think was a whole lot.
Ranking high is a total crap shoot, and greatly depends on your competition. For instance... currently I have two sites to work on. One site has very little competition, and the other is retail (very high comp). Gee... take a guess at which one ranks 50th on google.
Take all of those items seriously, build heavy content (not repetitive), and get links... you should then be alright.

All of the above mentioned are important and part in doing SEO.. The thing here now is on how will you going to do in the right way.. ? There are still some good tricks in doing those things.. and i guess, working hard for it is just one major thing in doing off page optimization for you to get your desired SERP's rankings..
